VBI (Vertebrobasilar Insufficiency) testing is a pre-manipulative screening procedure used by clinicians, such as physiotherapists, to evaluate blood flow through the vertebral artery to the brain, aiming to detect potential risk for stroke or arterial insufficiency before performing neck manipulation. It involves placing the patient's neck into, or near, end-range extension and rotation—often with added pressure—to compress the artery and check for symptoms of ischemia like dizziness, vertigo, or diplopia.
The goal of pre-manipulative vertebrobasilar testing is to evaluate the adequacy of blood supply to the brain by compressing the vertebral artery and to examine for the onset of signs and symptoms indicative of VBI.
Vertebrobasilar insufficiency (VBI) is reduced or stopped blood flow to the back of your brain. Hardening of your arteries (atherosclerosis) often causes this condition. VBI impacts your balance and movement, as well as increases your risk of stroke and other health issues.
It causes ischemia, due to blood loss in the pons and the medulla oblongata of the brain. This results in dizziness, nausea, syncope, dysarthria, dysphagia, and disturbances of the hearing or vision, paresis, or paralysis of patients with Vertebrobasilar Insufficiency (VBI).
Symptoms of vertebrobasilar insufficiency
Double vision. Vertigo (spinning sensation) Numbness or tingling. Nausea and vomiting.
Doppler ultrasonography and magnetic resonance imaging (MRI) are two important measures for diagnosis of VBI (5,6). On T2 weighted noncontrast axial MRI sequences, “slice-entry phenomenon” can exist due to the variability of blood flow in vertebral arteries (3).
If blood flow to the brain's balance centers (like the cerebellum, brainstem, or inner ear) is compromised due to a Transient Ischemic Attack (TIA) or a stroke, vertigo can be a prominent and serious symptom. Patients with PAD are at a significantly higher risk for TIA/stroke due to this widespread arterial disease.
Symptoms

Atherosclerosis or "hardening of the arteries" is the main cause of vertebrobasilar disease. The narrowing of the vertebral or basilar arteries caused by atherosclerosis creates vertebrobasilar insufficiency (VBI), or an insufficient delivery of blood flow to the posterior structures of the brain.
When this blood supply is insufficient, vertebrobasilar insufficiency (VBI) can develop and cause symptoms such as neck pain, headaches/migraines, vertigo, dizziness, cardiovascular-like problems, drop attacks, difficulty swallowing, and/or speaking, and auditory and visual disturbances.
Overview. Intracranial stenosis, also known as intracranial artery stenosis, is the narrowing of an artery in the brain, which can lead to a stroke. The narrowing is caused by a buildup and hardening of fatty deposits called plaque. This process is known as atherosclerosis.
VBI exhibits a characteristic symptom constellation: nausea, vertigo, nystagmus, cerebellar signs such as ataxia and dysmetria, ipsilateral cranial nerve deficits with contralateral corticospinal involvement, dysphagia, dysarthria, and occipital lobe-related visuospatial or visual field deficits.
The vertebrobasilar (VB) system, comprised of the vertebral and basilar arteries, serves as a critical arterial supply to the cervical spinal cord brainstem, cerebellum, thalamus, and occipital lobes.
Arterial insufficiency symptoms
There are four carotid arteries, with a pair located on each side of the neck. This includes the right- and left-internal carotid arteries, and the right- and left-external carotid arteries. The carotid arteries deliver oxygen-rich blood from the heart to both the head and brain.
You can check your carotid artery at home by gently feeling for the pulse on either side of your windpipe (trachea) using your index and middle fingers, applying light pressure in the groove next to the trachea and sternomastoid muscle, but never press both sides at once, as it can cut off blood flow to the brain. While this lets you check pulse rate and strength, it doesn't reliably detect blockages, so see a doctor for concerns, especially if you notice symptoms like dizziness, numbness, vision changes, or weakness, as only professional tests like Doppler ultrasound can confirm carotid artery disease.
The first signs of a blocked carotid artery often mimic a stroke (Transient Ischemic Attack or TIA) and include sudden numbness or weakness (especially on one side), vision problems (blurred vision, temporary blindness in one eye), difficulty speaking or understanding, dizziness, or a severe, sudden headache, but sometimes there are no symptoms until a full stroke occurs, highlighting the importance of risk factor management and checkups.
Techniques and procedures used to improve blood flow to the brain are similar to those used in heart procedures. They include bypass surgery, balloon angioplasty and stenting. One of the latest devices is called a stent retriever (also known as a stentriever).
Early signs of vascular dementia often involve slowed thinking, problems with organization, planning, and concentration, and difficulty finding words, sometimes appearing suddenly after a stroke or ministrokes, rather than primarily memory loss like in Alzheimer's. You might also see changes in mood (depression, apathy), restlessness, an unsteady walk, or issues with bladder control, varying based on the brain area affected.
Hearing disorders in the carotid vessel diseases are characterized by high volatility. They may manifest as progressive or sudden hearing loss or even sudden deafness. According to many authors, the failing function of the inner ear can appear as an isolated symptom a few days before stroke [15].
